New Patients
Whatever your dental needs, an entire range of dental care is available at the Dental Offices at Elizabeth Place. All new patients receive an oral examination to detect existing or potential problems.
Please plan to arrive 30 minutes early for your first visit to the the Dental Offices at Elizabeth Place in order to complete a patient registration form (click to download) and medical history. If you have recent dental x-rays you may bring them with you. If you are being referred to Dr. Sato from another dentist please bring your referral form or letter.
The Dental Offices at Elizabeth Place are open for appointments Monday 8am-4pm, Tuesday and Wednesday 10am-6pm, and Thursday 8am-2pm. Our staff will be happy to schedule an appointment for you. From your first contact with our front desk staff all the way through to your time sitting in the dental chair, we strive to make your experience a pleasant one. Our staff will do everything possible to assure you get the dental attention you need. All costs will be reviewed before treatment is rendered. For your convenience, we accept Visa, Discover, American Express, and Master Card. We also offer third party financing through CareCredit (click for link). We can assist you in applying for third party financing in our office once a treatment plan is established. More information about third party financing is available at the links below. In order to deliver excellent care at the most reasonable prices, payment is due at the time services are rendered.
We accept most dental Insurance Plans. We are in network with Superior, United Concordia, Delta Dental Premier and Cigna. Whether you come with an insurance we are in network with, or an out of network plan, we are happy to file your claims for you. We will provide your insurance company with anything they need to assure you receive the maximum benefit possible. Any balance after your insurance has paid is your responsibility.
The federal law requires us to provide our patients with information regarding our Privacy Practices. The Notice of Privacy Practices (click to download), contains this information. You will be asked to sign a form stating we made you aware of our Notice of Privacy Practices at the time of your initial visit.
